Abstract
The bilateral upward plus downward gaze palsy is a rare neurological manifestation caused by vascular insult, because it happens due to bilateral vascular lesions. We experienced a 78-year-old patient presenting diplopia and dizziness due to vertical gaze palsy. The magnetic resonance imaging of brain showed acute ischemic lesions in bilateral rostral paramedian midbrain, including rostral interstitial nucleus of the medial longitudinal fasciculus. And the magnetic resonance angiography didn¡¯t show left first branch of posterior cerebral artery.
